  • 臨床心理学、特に認知行動理論・療法に関する研究を行っている。これまで、抑うつを維持・悪化させる心理的要因に関する検討や、それらの知見を活かした、うつ病患者への集団認知行動療法の効果について研究を行ってきた。また最近は、若年労働者の就職後のストレス(リアリティ・ショック)など働く人のメンタルヘルスについても研究を進めている。

      Introduction

      The COVID-19 pandemic has led to increased social isolation for mothers, and rumination exacerbates postpartum depression in mothers with poor social support. Although behavioral activation can help to decrease their depressive symptoms, the mechanism by which behavioral activation reduces postpartum depression remains unclear.

      Methods

      We examined the effects of rumination and behavioral activation on depression in postpartum women by examining a model mediated by subjective reward perception. A questionnaire was administered to 475 postpartum women (Age: Mean = 30.74 years, SD = 5.02) within 1 year of childbirth using an Internet survey. The measurements included perinatal depression, rumination, and behavioral activation, and we assessed environmental reward. To control for confounding variables, we assessed psychiatric history, social support, parenting perfectionism, and COVID-19 avoidance.

      Results

      Eighty-four (17.68%) mothers had possible postpartum depression. The covariance structure analysis showed that not only was there a direct positive path from rumination to postnatal depression but also a negative path via reward perception.

      Discussion

      This finding indicated that the COVID-19 pandemic could have increased depression in many of the mothers. Rumination not only directly relates to postpartum depression, but it could also indirectly relate to postpartum depression by decreasing exposure to positive reinforcers. In addition, having a history of psychiatric illness increases the effect of rumination on postpartum depression. These findings suggest that psychological interventions are needed to reduce rumination and increase contact with positive reinforcements to reduce postpartum depression, especially for high-risk groups.

      Abstract

      Aim

      The aim of this research was to create a scale to assess the competency of therapists who conduct group cognitive behavioral therapy (G‐CBT). The scale is intended to serve as a tool to aid the training of therapists.

      Methods

      Three stepped studies were conducted. Process 1: Through literature review and experts' consensus process, essential skills for G‐CBT were articulated and categorized according to the criteria of the Cognitive Therapy Scale, a well‐established rating scale for evaluating clinicians' skills in individual cognitive behavioral therapy. The list of those skills was organized into a rating scale. Process 2: Behavioral anchors were added to each skill and were classified by the levels of difficulty (beginner, intermediate, and advanced levels), based on the rating by G‐CBT experts. Process 3: Inter‐rater reliability and validity of the rating scale were examined in a sample of 41 videotaped G‐CBT sessions of actual clinical sessions and educational role‐plays.

      Results

      The 12‐item Group Cognitive Therapy Scale (G‐CTS) was developed. It consists of 11 items that are adapted from the original Cognitive Therapy Scale, and a new 12th item called “Intervention using relationships with other participants,” which describes therapists' skills to address group dynamics. The G‐CTS showed excellent internal consistency (Cronbach's α: 0.95), satisfactory inter‐rater reliability (interclass correlation coefficients: 0.65–0.88), and high predictive validity.

      Conclusion

      A novel rating scale to evaluate therapists' competency in G‐CBT was developed and successfully validated. The G‐CTS behavioral checklist created in this study provides concrete guidelines that can be used by therapists to hone their skills in G‐CBT.

      The purpose of this study was to develop a Japanese version of the Work-related Acceptance and Action Questionnaire (WAAQ) and to examine its reliability and validity. In Study 1, we created a Japanese version of the WAAQ, examined its structural validity and internal consistency, and conducted hypothesis testing (convergent validity) using 180 workers. The Japanese version of the WAAQ was found to have high internal consistency, partial structural validity, and convergent validity. In Study 2, we examined the structural validity of the questionnaire and conducted hypothesis testing (convergent validity and comparison between subgroups) using 2,071 workers, of which 320 were involved in verifying test-retest reliability and measurement error. The results showed that the Japanese version of the WAAQ possessed high convergent validity and adequate structural validity, and displayed small effects in difference between subgroups in terms of age group, type of business, job type and measurement error. However, test-retest reliability remained an issue.

      <p>Rumination has been associated with impaired attentional control. People with ruminative tendencies may experience problems in everyday life because they fail to divide attention between internal and external information; they must cope with external events even as they become trapped in rumination. To clarify the characteristics of negative rumination, we investigated the relationship of negative rumination ("negative rumination trait" and "uncontrollability of negative rumination") with divided attention between internal and external information. Fifty-eight university students performed a divided attention task (dual task) consisting of (1) a primary, external attention task, in which they were asked to press a button corresponding to the location of an asterisk displayed on a computer screen, and (2) a secondary, internal attention task, in which they were asked to retrieve words from memory. The results showed that the capacity of divided attention was correlated with "negative rumination trait" in men and with "uncontrollability of negative rumination" in women, suggesting that people with higher ruminative tendencies had more difficulty dividing attention between internal and external information.</p>

      BACKGROUND: Depression is characterized by negative self-cognition. Our previous study (Yoshimura et al. 2014) revealed changes in brain activity after cognitive behavioral therapy (CBT) for depression, but changes in functional connectivity were not assessed. METHOD: This study included 29 depressive patients and 15 healthy control participants. Functional Magnetic Resonance Imaging was used to investigate possible CBT-related functional connectivity changes associated with negative emotional self-referential processing. Depressed and healthy participants (overlapping with our previous study, Yoshimura et al. 2014) were included. We defined a seed region (medial prefrontal cortex) and coupled region (ACC) based on our previous study, and we examined changes in MPFC-ACC functional connectivity from pretreatment to posttreatment. RESULTS: CBT was associated with reduced functional connectivity between the MPFC and ACC. Symptom change with CBT was positively correlated with change in MPFC-ACC functional connectivity. LIMITATIONS: Patients received pharmacotherapy including antidepressant. The present sample size was quite small and more study is needed. Statistical threshold in fMRI analysis was relatively liberal. CONCLUSIONS: CBT for depression may disrupt MPFC-ACC connectivity, with associated improvements in depressive symptoms and dysfunctional cognition.

      うつ病に対する認知行動療法の効果についてはエビデンスが蓄積されてきているが,その神経作用メカニズムについては,依然として明らかではない。本稿では,Disnerらの提唱したうつ病の認知モデルに関して推測される神経メカニズムを中心に,うつ病におけるネガティブな情動刺激の処理や自己関連づけ処理に関する神経メカニズムを紹介した。そして,うつ病の認知行動療法の神経作用メカニズムに関して,DeRubeisらによる安静状態の画像研究の結果に基づくモデルとYoshimuraらの研究を紹介した。特に,Yoshimuraらの研究は,自己関連づけ処理のように特定の認知機能に関連した神経ネットワークへの影響を検討したものであり,認知行動療法の神経作用メカニズムに迫る研究といえる。今後,更にエビデンスが蓄積されることで,認知行動療法がどのような認知機能に関連した神経ネットワークを変容させるのかが明らかになると期待できる。最後に,治療反応予測研究についても紹介した。うつ病治療に関するさまざまなエビデンスが蓄積されるなかで,うつ病患者に対して個別化された治療法選択に関する研究が今後期待される。(著者抄録)

      Cognitive behavioral therapy (CBT), an effective treatment for depression, targets self-referential processing of emotional stimuli. We examined the effects of CBT on brain functioning during self-referential processing in depressive patients using functional magnetic resonance imaging (fMRI). Depressive patients (n = 23) and healthy participants (n = 15) underwent fMRI scans during a self-referential task using emotional trait words. The depressive patients had fMRI scans before and after completing a total of 12 weekly sessions of group CBT for depression, whereas the healthy participants underwent fMRI scans 12 weeks apart with no intervention. Before undergoing CBT, the depressive patients showed hyperactivity in the medial prefrontal cortex (MPFC) during self-referential processing of negative words. Following CBT, MPFC and ventral anterior cingulate cortex (vACC) activity during self-referential processing among depressive patients was increased for positive stimuli, whereas it was decreased for negative stimuli. Improvements in depressive symptoms were negatively correlated with vACC activity during self-referential processing of negative stimuli. These results suggest that CBT-related improvements in depressive symptoms are associated with changes in MPFC and vACC activation during self-referential processing of emotional stimuli.

      IntroductionThe aim of this paper is to report on the Rework Program, a type of high-level rehabilitation program developed in Japan, and on Cognitive-Behavioral Group Therapy for Rework (CBGTR), which was also developed in Japan.MethodsWith regard to the Rework Program, we first discuss conceptual issues, including the need for the Rework Program, rework identity, destigmatization as a desired outcome, and the perceived significance of the Rework Program. Its purpose, historical background, framework, criteria for participation, staffing, program categories, and assessment issues are then described. With regard to CBGTR, this paper describes its purpose, historical background, structure, staffing, content, shared themes, and group advantages.ResultsAs of 30 June 2010, 81 facilities in Japan offer the Rework Program; 55 of these facilities also provide CBGTR.DiscussionIn Japan, the Rework Program and CBGTR are established practices. Systematic program standardization and effectiveness studies are currently being performed. These innovations in Japan may provide insight and ideas to colleagues in other regions.

      BACKGROUND: Depression is characterized by enhanced self-referential processing of negative emotional stimuli. Imaging studies have suggested that activation of both the medial prefrontal (MPFC) and anterior cingulate cortices (ACC) is associated with self-referential processing. However, whether this pattern of activation occurs in depressed individuals during the self-referential processing of the emotional stimuli had not been investigated to date. METHODS: Participants were 13 patients with major depressive disorder and 13 normal controls. We used block-designed functional magnetic resonance imaging (fMRI) to investigate neural activity during the self-referential judgments of positive and negative valenced personality trait words. RESULTS: Compared with the normal controls, the depressed patients showed hyperactivity in the MPFC and the rostral ACC during the self-referential processing of negative words. In addition, the activity of these regions during self-referential processing of the negative stimuli was correlated with the depressive symptom severity. The rostral ACC activity mediated the correlation between the MPFC activity and the depressive symptoms. Functional connectivity analysis revealed positive connectivities between the MPFC, the rostral ACC, and the amygdala. LIMITATION: Small N and antidepressant effect on imaging data limit the stability of reported findings. CONCLUSIONS: The relationships between the MPFC, the rostral ACC, and the amygdala appear to reflect an interaction between the self-referential processing and the negative emotional information processing, and we propose that the strong connection between the MPFC and the rostral ACC is associated with depressive symptoms.

      BACKGROUND: Although patients with Treatment Resistant Depression (TRD) often have impaired social functioning, few studies have investigated the effectiveness of psychosocial treatment for these patients. We examined whether adding group cognitive behavioral therapy (group-CBT) to medication would improve both the depressive symptoms and the social functioning of patient with mild TRD, and whether any improvements would be maintained over one year. METHODS: Forty-three patients with TRD were treated with 12 weekly sessions of group-CBT. Patients were assessed with the Global Assessment of Functioning scale (GAF), the 36-item Short-Form Health Survey (SF-36), the Hamilton Rating Scale for Depression (HRSD), the Dysfunctional Attitudes Scale (DAS), and the Automatic Thought Questionnaire-Revised (ATQ-R) at baseline, at the termination of treatment, and at the 12-month follow-up. RESULTS: Thirty-eight patients completed treatment; five dropped out. For the patients who completed treatment, post-treatment scores on the GAF and SF-36 were significantly higher than baseline scores. Scores on the HRSD, DAS, and ATQ-R were significantly lower after the treatment. Thus patients improved on all measurements of psychosocial functioning and mood symptoms. Twenty patients participated in the 12-month follow-up. Their improvements for psychosocial functioning, depressive symptoms, and dysfunctional cognitions were sustained at 12 months following the completion of group-CBT. CONCLUSIONS: These findings suggest a positive effect that the addition of cognitive behavioural group therapy to medication on depressive symptoms and social functioning of mildly depressed patients, showing treatment resistance.

      うつ病に対する集団認知行動療法(CBGT)は、薬物療法との併用で抑うつ症状の改善や再発予防に有効であることが報告されているが、わが国では実証的研究は行われていない。そこで、広島大学病院ではうつ病のCBGTプログラムを作成し、1グループ5〜6名のうつ病患者を対象に、12セッションのプログラムを施行した。さらに、CBGT前後で抑うつ症状、心理・社会的機能を評価し、CBGTの有効性について検討してきた。その結果、うつ病のCBGTは患者間のサポート機能、教育機能、強化機能が充実しており、抑うつ症状や心理・社会的機能の改善に有効であることが明らかにされた。(著者抄録)

      喫煙妊婦の再喫煙率は高く、周産期合併症、周産期うつ発症のリスクのみならず次世代の乳児突然死症候群を代表とする健康被害は明らかであり、科学的根拠のある妊婦禁煙支援は急務である。申請者は、ICTを活用し禁煙への行動変容を支援するためにに、妊婦禁煙アプリケーション(以下、アプリ)の開発を目指している。喫煙妊婦の特性として、禁煙治療薬を使用できないこと、ヘルスリテラシーの低さや健康無関心層が多い事から、様々な創意工夫が必要である。
      本研究の目的は、(1)喫煙妊婦に対する禁煙と再喫煙予防のためのアプリを開発する(2)アプリを用いた禁煙成果の有用性を検証する(3)長期的な禁煙継続が産後うつの予防となるかについて検討することである。令和3年度は、女性喫煙者の特徴、禁煙動機、再喫煙のリスク、女性や妊婦に有効な禁煙支援をもとに、認知行動療法を取り入れた禁煙プログラムを開発する事を予定していた。アプリは、各行動変容モデルに沿った情報提供、セルフケア力の強化、喫煙による母体や胎児、出生児、家族への影響、喫煙パターンの理解(喫煙の引き金と対処)、ストレス対処の実行、禁煙開始日の備え(周囲のサポートと努力の可視化)、再喫煙の前兆への気づきと予防策(代替行動)、妊婦健康診査時の活用法、副流煙対策、喫煙家族の禁煙推奨、体重管理、質疑応答機能、日記機能のコンテンツの開発過程である。
      行動変容無関心層への対応についても検討を重ね、行動変容の各時期に沿った情報提供やアウトリーチ機能をコンテンツに加えた。さらに、ヘルスリテラシーの低い対象者への配慮として、可能な限り、専門用語を避けた、絵や写真、ノモグラムを使用したアプリを開発中である。

      The purpose of this study is to construct the comprehensive support system for the prevention of the mental health aggravation of new teachers. This study is examined following three points mainly;(1) We elaborate a mental health training program as the self-care education for the reality-shock.(2) We show influence to give to the mental health of the new teacher from the viewpoint of social capital about the resource of interaction with the managerial class and the life scene using qualitative and quantitative examination.(3) Based on the knowledge of 2 sides mentioned above, we clarify a useful support method for reality-shock reduction depending on the type of the new teacher and return it to the local government and the school spot.

      This study investigated the phenomenon of reality shock experienced by first-year thechers amd examined the effect of a prevention program. We conducted cross-sectional research and longitudinal research to determine whether the reality shock experienced in teaching affects first-year teachers's mental health. The cross-sectional research showed a significant correlation between reality shock related teaching adn stress response among first-year teachers. Moreover, the longitudinal research suggeted that reality shock significantly predicted stress response. In addition, first-year teachers underwent a mental health program that focused on coping with reality shock; we compared the program's effects on these teachers with those observed in a control group. The efficacy of the programn was not confirmedl but the program showed potential for use in the improvement of cognitive coping.

      We investigated the psychosocial disability and the effectiveness of psychosocial treatment in patients with treatment-resistant depression(TRD).       本研究の目的は,うつ病に対する集団認知行動療法プログラムの効果を検討することである.平成18年度は,昨年度に引き続き,うっ病患者を対象に集団認知行動療法プログラムを実施し,その短期的効果を検討するとともに,コントロール群データを収集し,集団認知行動療法群との比較検討を行った.
      対象者は,うつ病患者18名(男性16名,女性2名.平均年齢43.6±8.5歳)であった.1名を除き,17名は薬物療法を併用した.集団認知行動療法プログラムは12セッション(約3ヶ月間)からなり,各セッションのねらいは心理教育,自己理解,思考の再検討,生活場面での実践,再発予防であった.1グループは患者4-6名で構成され,スタッフ3名でプログラムを実施した.さらにプログラム前後に,抑うつ症状(BDI, HAM-D),社会的機能(SF-36),非機能的認知(ATQ-R, DAS)を測定した.一方,コントロール群は薬物治療中のうつ病患者6名(男性2名,女性4名.平均年齢44.5±9.1歳)であった.コントロール群には,質問紙調査(BDI, SF-36,ATQ-R, DAS)を約3ヶ月の間隔をおいて2回実施した.
      まず集団認知行動療法群18名の抑うつ症状および心理・社会的機能について,時期を要因とした分散分析を行った.その結果,BDI, HAM-D, SF-36「身体機能」「活力」「社会生活機能」,ATQ-R「肯定的思考」について改善が認められた.次に,集団認知行動療法群とコントロール群の各指標の変化量について,群を要因とした分散分析を行った結果,いずれの指標も群の差は認められなかった.
      以上のことから,うっ病の集団認知行動療法プログラムと薬物療法の併用は,抑うつ症状や心理・社会的機能の改善に有効であることが確認されたが,その効果は薬物療法単独に比べて優れているとまではいえなかった.今後はさらにサンプル数を増やして検討する必要がある.
