    立教大学文学部・文学研究科博士課程前期課程で西洋史を学び、その後ロンドン大学ウェルカム医学史研究所にて博士号(医学史)を取得。19~20世紀イギリスにおける精神医療の歴史を中心に研究を行ってきた。近年は、戦争と医学、痛みや感覚の歴史、体温計測の歴史、上水道と鉛中毒の歴史、医療アーカイブズ論、医療ミュージアム論などに研究関心を広げている。主たる業績に、『精神医療、脱施設化の起源-英国の精神科医と専門職としての発展1890-1930』(みすず書房、2017年)、"Surviving the Lunacy Act of 1890: English Psychiatrists and Professional Development during the Early Twentieth Century", Medical History, 61(2), 2017, pp. 246-269などがある。

      This paper aims to explore an environmental aspect to mental disease in modern England, paying particular attention to lead exposure through the water supply. People living in Western Europe were increasingly exposed to chemical materials to a great extent both in workplaces and at homes particularly after industrialization. Among them, lead, a poisonous heavy metal, has been widely employed from water pipes and cisterns, soldered cans, to colouring for ceramics and toys. Lead thus pervaded the lives of modern humans in numerous ways. However, lead could seriously affect human health, causing damage to nerves, blood cells, the liver, kidney and reproductive organs. If severely exposed to lead, human beings show symptoms such as anemia, abdominal pain, constipation, paralysis and mental depression and delusions, sometimes resulting in death. Even a small amount of lead exposure, such as 0.1 mg/Lin blood or 0.01 mg/Lin drinking water, would increase the risk of delays in nervous and intellectual development, of learning disabilities and of schizophrenia, as reported by medical reports published in the latter part of the twentieth century. Historians of the environment, occupational diseases, water supply and public health have referred to lead exposure, but most of them described it as a minor and temporal environmental problem that had been overcome in the past. This historical perspective was questioned by Werner Troesken, an American socio-economic historian. He argued in his seminal book The Great Lead Water Pipe Disaster published in 2006 that in modern Britain and the United States, lead exposure through leaden water pipes caused more serious health damage than had been thought, and its extent was particularly lethal in Massachusetts and northern England. As Erik Millstone, a well-known researcher of social policy in England, revealed, such serious lead exposure has continued in the British Isles. Meanwhile from the 1970s to 2010s, medical researchers paid more attention to the relationship between lead exposure and mental diseases. A particularly important study showed that prenatal exposure to lead, even at modest levels, would increase the risk of schizophrenia 1. 8 times. This finding remained outside the attention of historians, except David Healy, an eminent British psychiatrist and a historian of psychiatry. In his 2014 paper, Healy proposed historians shift their attention to environmental issues. Following Healy's suggestion, this paper attempts to study health damage caused by lead exposure, and the relationship between lead exposure and mental disease, in northern England, especially in the West Riding of Yorkshire (currently West Yorkshire), and its major city Sheffield between the early nineteenth century and the early twentieth century. The West Riding of Yorkshire has been known as an area in which lead poisoning spread significantly, and Sheffield was the heart of lead poisoning in the late nineteenth century. Historians have barely studied these events, nor examined the actual extent of lead exposure and its health effects. What was the extent of lead exposure through the water supply in nineteenth century Britain? How did people living in the West Riding of Yorkshire and Sheffield experience lead exposure? How serious was the lead exposure? Armed with these questions, this paper first inquires into the general history of lead exposure in Britain from ancient times to current times, and secondly introduces medical and psychiatric studies of lead exposure and its relation to mental diseases developed particularly after the late nineteenth century. It then refers to the history of water supply in England, including the popularization of leaden water pipes and its sub-effects on human health and focuses on local experiences in the West Riding of Yorkshire and Sheffield. Employing both qualitative documents (local newspapers and administrative reports) and a series of quantitative documents (Census and various statistics produced by the Registrar-General's Office, local governments and local mental hospitals), this paper concludes that on one hand people experienced serious, sometimes lethal, lead exposure through the water supply in the West Riding of Yorkshire and Sheffield in the late nineteenth century, and, on the other hand, that such serious exposure shows no clear correlation to the occurrence of mental diseases. This curious finding should lead us to a further research in the future because it suggests other factors are linked to mental disease. They would probably be poverty, living and working conditions, or nutrition, all of which seem to be related to modern ways of living.

      Osamu Kanamori (1954-2016) was a prolific author of science and technology studies in Japan in the late twentieth and early twenty-first centuries. He represented many new directions, which he originally learned from France, the United States, and Japan. He influenced histories of scientific ideas and STS in Japan and East Asia. Around the same period, a new history of medicine in modern Japan started to take off. Many historical studies of modern medicine in Japan are published in English. This special issue tries to examine the relationship between Kanamori's works and Japanese medical historians and medical sociologists. After extensively reviewing and summarizing the variety of themes and genres within the works of Kanamori, these four papers will discuss four topics of medical technology, infectious diseases, psychiatric war pensions, and bioethical sci-fi works which have all been inspired by the works of Kanamori. This special issue explores the extensive works of Kanamori and the new history of medicine in Japan and argues that the new history of medicine in the near future becomes a core academic discipline within the relationship of the philosophical discussion of medicine in society and culture.

      Modern medicine has developed a number of technologies to observe, diagnose, and intervene in the human body. Medical technologies make bodies visible by number and form. Many historians have studied how such medical technologies as the stethoscope, pulse meter, cardiograph, and thermometer were invented and subsequently influenced societies. However, recent historians have turned their attention to the technology-in-practice to show a unidirectional history intertwined by devices, clinicians, and users. This paper aims to follow this line of study by focusing on a history of clinical thermometry in modern Britain and Japan. First, it surveys secondary works on medical technologies in history and explores how clinical thermometers were invented, improved, sold, and consumed in two distinct cultures in Britain and Japan. It argues that clinical thermometry was used for consumers' own sakes, for example, performing a gender role and dreading influenza, and also framed by such external factors as war and pandemics.

      In recent decades, historians of English psychiatry have shifted their major concerns away from asylums and psychiatrists in the nineteenth century. This is also seen in the studies of twentieth-century psychiatry where historians have debated the rise of psychology, eugenics and community care. This shift in interest, however, does not indicate that English psychiatrists became passive and unimportant actors in the last century. In fact, they promoted Lunacy Law reform for a less asylum-dependent mode of psychiatry, with a strong emphasis on professional development. This paper illustrates the historical dynamics around the professional development of English psychiatry by employing Andrew Abbott's concept of professional development. Abbott redefines professional development as arising from both abstraction of professional knowledge and competition regarding professional jurisdiction. A profession, he suggests, develops through continuous re-formation of its occupational structure, mode of practice and political language in competing with other professional and non-professional forces. In early twentieth-century England, psychiatrists promoted professional development by framing political discourse, conducting a daily trade and promoting new legislation to defend their professional jurisdiction. This professional development story began with the Lunacy Act of 1890, which caused a professional crisis in psychiatry and led to inter-professional competition with non-psychiatric medical service providers. To this end, psychiatrists devised a new political rhetoric, early treatment of mental disorder', in their professional interests and succeeded in enacting the Mental Treatment Act of 1930, which re-instated psychiatrists as masters of English psychiatry.

      本稿は、ミシェル・フーコーの規律化と統治性に関する議論を念頭におきつつ、20世紀前半のイングランドにおける精神病院とその患者の問題を検討するものである。フーコーは、18 ~ 19世紀のヨーロッパにおける精神病院の勃興について、非理性の代表格たる狂気を規律化し、理性を持つ者の側に復帰させるための啓蒙主義的試みとして論じた。このフーコーの議論をめぐっては、実証的な歴史学の立場から再検討が加えられ、実際の精神病院の現場では精神病者とその家族の利害が考慮されていたことが明らかとなった。しかし、こうした実証的な研究は、20 世紀の精神病院とその患者たちを視野の外に置いていた。それは、20 世紀の精神病院には19 世紀とは異なる特質が認められるためであった。19 世紀末になって狂気の規律化が失敗に終わりつつあることが徐々に認識されると、精神病院という施設を通じた規律化を高コストなものとして退け、ソーシャル・ワークを中心とした施設外での取り組みが増えていった。このような歴史的展開は、フーコーが「生権力」「統治性」と呼んだ概念の下でより鮮明に理解することができる。フーコーは、近代社会の特徴を、集団レベルでの生命の特性を把握し、その調整を行う権力である生権力、人口集団を政治経済的に統制するための様々な制度や戦術の動員を意味する統治性という二つの概念の下で論じた。つまり、フーコーは、規律化とは異なる管理と統治の技法の存在を示唆している。本稿は、その新たな管理と統治の技法が実際の精神医療の現場においても確認できるものかを問うものである。具体的には、ロンドン近郊に所在したクライバリ精神病院の運営委員会記録を分析し、20世紀前半の精神病者たちは果たして、生権力と統治性という、いわば精神医学の権力に服する存在だったのか。彼ら自身の主体性は認められないのかを検討した。分析の結果、精神病院と精神科医たちは多くの場合、患者とその家族の利害を汲んでいたことが明らかとなった。ただし、フーコーが論じた別の概念、統治手段としての家族、あるいは司牧的権力論を参照すると、患者の主体性を認めることは一概には望ましくないことも確認された。結論としては、20 世紀前半のイングランドにおける精神医療は、ソーシャル・ワークという新たなサービス形態を通じて、患者とその家族の生活へとアプローチし、そのチャンネルを通じた国民生命と健康の管理を目指したことが論じられた。

      20世紀を通じて世界各国は、入院型の精神病院を中心としたものから外来設備を核とし地域に根ざした精神医療、すなわちコミュニティ・ケアへと、医療サービスの形態を変化させてきた。これは精神医療の脱施設化と呼ばれる現象であるが、英国では19世紀末から始まっていた。 1890年、イングランドでは狂気法と呼ばれる法律が成立した。この法律は、精神疾患を患っていない人々が強制的に精神病院へと監禁された事件を背景とし、患者の人権擁護を目指したものだった。 しかしイングランドの精神科医たちは、狂気法の非人道性や治療上の非効率性など、入院型の精神病院によらない新たな精神医療の形態、脱施設化された精神医療を提案していった。なぜ、精神科医たちは1890年狂気法に反対し、脱施設化された精神医療の必要性を訴えたのか。 本書は、この問いに答えるために、精神科医という専門職に注目し、その政治的言説、職階構造、診療実践、病院経営、他専門職との競争のありかたを検討する。そこからは、社会学者A・アボットが論じた、近代西洋社会に特徴的な専門職発展の歴史的様相が確認される。精神医療の脱施設化は、精神科医=専門職の政治経済的な戦略と社会的な諸関係なくしては現われることはなかった、歴史上の現象であった。

      本共同研究の成果公表の一つとして、研究分担者5名が研究成果を2015年7月4日に総合研究大学院大学(葉山)で開催された化学史学会年会シンポジウム「近代イギリスにおける科学の制度化:イギリス史研究の視点から」で発表した。すなわち、大野誠の「趣旨説明」に続いて、坂下史が「地方都市における農業協会の活動と草の根啓蒙」、石橋悠人が「国営天文台と科学の制度化: 19世紀のグリニッジ天文台を事例に」、高林陽展が「医学研究委員会から医学研究評議会へ: 大戦期の経験と医学研究の制度化」、奥田伸子が「ノーベル賞を受賞した「主婦」: 20世紀中葉イギリスにおける女性科学者と社会」と題して報告した。
      次の研究分担者は,それぞれが関与する学会や学術集会で本テーマに関わる研究成果を発表した。松波京子は、オランダで開催された第14回国際18世紀学会、第8回日英歴史家会議、進化経済学会で、伊東剛史はKrakowで開催されたICC-TUFS Joint Seminarやイギリス女性史研究会、歴史学研究会近代史部会で、石橋悠人は政治経済学・経済史学会と19世紀学学会で発表した。
      本共同研究内部でテーマについて理解を発展させる目的で、本年度は「公共圏」概念を取り上げ、国内の研究者2名を招聘して発表を行なってもらい、討論した。2016年1月9日に岩間俊彦(首都大学)氏が「討論による市民文化の形成: バーミンガムにおける討論団体1850-1890年、ジョン・ブライトンに関する討論を中心に」、3月13日に田中拓道(一橋大学)氏が「公と民の対抗から協調へ: 19世紀フランスの福祉史」について発表し、討論を行った。いずれも会場は愛知県立大学サテライトキャンパス(名駅)で、名古屋近代イギリス研究会にも参加を呼びかけた。
      イギリスでの史料調査を坂下史、高林陽展、椿達也が実施し、国際学会での発表を松波京子、菊池好行が行った。

    • 近現代イギリスにおける身体観・疾病観・死生観 研究課題

      本研究の目的は、19世紀後半から20世紀前半のイギリスにおける身体観・疾病観・死生観の変化を分析することである。この時期のイギリスでは、コレラや結核といった感染症の死亡率が低下し、社会の広い層で相対的な健康が実感されつつあった。その結果、人々は、以前は気にしなかった微細な身体の失調を問題視するようになった。また、感染症ではなく、生活習慣病や慢性疾患へと関心を寄せていった。その具体的な事例として、本研究では、体温計の利用法について検討を進めてきた。体温計は1870年代に携帯できる小型商品が実用化され、一般家庭にも広がりを見せた。これによって、人々は日々の体調の変化を自ら観察できるようになったのである。この点に関する、2015年度の主たる研究成果は、2015年8-9月と2016年2-3月に現地調査を行い、その調査結果をもって、2016年医療の社会史学会(Society for Social History of Medicine)大会の口頭発表に応募し、採択となったことである。現地調査においては、ウェルカム医学史図書館、大英図書館、ハックニー史料館、北ヨーク史料館、ブーツ史料部門などで各種一次史料を収集した。特に重要な一次史料としては、ハックニー史料館所蔵の医学機器製造会社キャセラの発注台帳を入手した点である。そこで得られた体温計購入者のデータを基に、北ヨーク史料館では利用者の実態分析に踏み込むことができた。また、イギリス最大の医薬品小売店であるブーツの史料部門では、小売店の体温計販売戦略を検討した。以上によって、製造業者、小売店、利用者が織りなす、健康維持の力学が徐々に明らかとなってきた。以上で述べたような、近代ヨーロッパの健康意識にかかわる歴史学的研究は乏しく、その意義を強調することができるだろう。

      医療アーカイブグループが推進する小峰研究所資料調査では、医療記録および組織資料の内容特定を進めている。医療記録の大部分を占める症例誌に関しては約8割程度の人名特定を達成した。組織資料の調査からは、会計帳簿や出金伝票が部分的に残存することが分かり、1ヶ月の経理状況の把握につながるなど、組織運営の実態把握に向けて大きく前進した。
      感染症・公衆衛生グループでは、「疾病と歴史」について上海の4大学から招待を受けての講演、「パンデミック時の日本および西欧における検疫をめぐる政府対応および社会の反応の比較史」等についての考察、「大正から昭和初期に於ける乳児死亡と感染症」や「スペインかぜ流行とわが国の衛生行政」等について分析と検討を進めた。
      ハンセン病グループでは、国立療養所菊池恵楓園所蔵「患者身分帳」に関する共同研究を同園倫理審査委員会の許可を得て実施してきた。分担して主に1920年代の「患者身分帳」の読解と分析を進めた。共通フォーマットに匿名化したデータを入力して情報を共有し、1907年法下でのハンセン病療養所の入所者に関して、共同して多角的な分析を加えた。
      精神疾患グループでは沖縄県公文書館および沖縄県立図書館が所蔵する戦後沖縄の精神衛生行政・公衆衛生看護に関する資料、京都癲狂院、東京府巣鴨病院、京都帝国大学ならびに東北帝国大学精神病学教室で行われていた音楽療法実践に関する資料、下総精神医療センターが所蔵する戦時下の国府台陸軍病院に関する資料を収集した。また、医療アーカイブズの保存と利用に関する議論を行った。
      国際拠点形成グループでは、国外出張が難しいため日本国内の施設における患者の情報を記録するメカニズムを調査した。王子脳病院の病床日誌や、九州帝国大学、東京帝国大学の記録の違い、日本の国内の言語上の異なりを調査し、日本語、ドイツ語、英語などが用いられるパターンを調べた。

      Hansen’s Disease and mental illnesses have allowed historians to analyze several major issues. Instead of the development of medicine by itself, nor of the experience of the patients, the relationship between the doctor (and other nursing staff) and the patients was important. Doctors and the patients were also affected by wider issues. Musical therapy incorporated the interests of both side; the family and the communities of the patients were crucial; science and technology was important for the doctors. As for tuberculosis, this project has so far failed to find rich patients’ records. We shall think about how to get an archive for tuberculosis and other infectious diseases.

      本研究は、20世紀中葉のイングランド精神医療における「地域医療」及び「脱施設化」を検討することを目的とし、研究活動を進めてきた。平成22年度においては、4月以降、国内での文献の収集と読解、論文の執筆を中心に研究を実施した。平成21年度に学術雑誌に投稿し、審査結果が得られた論文「第一次世界大戦期イギリスにおける戦争神経症」については、修正の上、7月に再投稿した。前年度二度にわたって実施した英国ロンドンでの現地史料調査(平成21年8月29日から9月12日、平成22年2月28日から3月17日)において、国立公文書館(National Archives)、ウェルカム財団医学史図書館(Wellcome Trust Library for Understanding Medicine)、大英図書館(British Library)で入手した史料を用いて数点の論考を準備した。その結果、査読論文「戦争神経症と戦争責任-第一次世界大戦期及び戦間期英国を事例として」が『戦争責任研究』から刊行され、査読論文「精神医療専門職の再検討-20世紀初頭イングランドにおける精神科医の職業構造を中心に-」が『精神医学史研究』にて平成23年度に掲載決定、査読論文「精神衛生思想の構築-二〇世紀初頭イングランドにおける早期治療言説と専門家利害-」が『史学雑誌』にて平成23年度に掲載決定となった。上記のように、平成22年度においては、主として研究成果の公表作業を進めることを主眼として、執筆活動を中心に行い、重要な成果を得た。
