      OBJECTIVE: Adequate conditioning results from various physical, environmental, and psychological factors in sports activities. In this study, we aimed to clarify the concurrence and relevance of injuries, psychological problems, and sleep disturbance in university football (soccer) players. Biomechanical characteristics and risk factors for those injuries were also investigated. METHODS: Overall, 1,084 university football players participated in this cross-sectional study using a web-based health history questionnaire. Assessments were made via the presence of injuries for physical problems, the Athlete Psychological Strain Questionnaire (APSQ) for psychologicalproblems, and the Athlete Sleep Screening Questionnaire (ASSQ) for sleep difficulties. RESULTS: There were 200 (19%) athletes with injuries; 413 (38%) of these were APSQ-positive, 246 (23%) were ASSQ-positive, and 468 (43%) athletes had no physical, psychological, or sleep problems. APSQ-positive respondents were significantly more likely to have a current injury than those who were APSQ-negative. No significant difference was observed between the number of ASSQ-positive and -negative respondents and current injuries. Those with sleep problems were significantly more likely to have psychological problems. CONCLUSION: More than 60% of university football players had one or more physical, psychological, or sleep problems. Players may have co-occurring problems, and it is important to address them for these athletes to reach peak performance.

      Injury prevention and health promotion among elite athletes are crucial to athletic success. Risk management requires the understanding and adjustment of physical, environmental, and psychological factors. Sleep status can be one of the essential factors for maintaining good health and preventing injury. The Athlete Sleep Screening Questionnaire (ASSQ) is expected to be an international standard for evaluating the sleep status of elite athletes. The 16 items ASSQ assess several sleep-related factors, including sleep difficulty, sleep-disordered breathing such as snoring and apnea, sleep and psychosomatic functioning during expeditions, chronotype, napping, caffeine intake, and electronic device use.

      A total of 158 male college soccer players responded the ASSQ-J and the Japanese version of the Pittsburgh Sleep Quality Index (PSQI-J). A factor analysis was conducted to validate the factor structure of the ASSQ-J. The internal consistency of ASSQ-J is assessed for validation. Pearson’s correlation coefficients and kappa statistic were calculated to determine if the ASSQ-J was significantly associated with PSQI-J respectively.

      Regarding the ASSQ-J-SDS, 22.4% (35 of 156) of the subjects in this study scored 8 or higher (moderate or higher), which was almost consistent with the results of a previous study on college athletes. It is clear that some Japanese male collegiate soccer players have sleep problems, as same as collegiate athletes around the world. Further research is needed for the validation of the ASSQ-J, the sleep status in a wider range of ages and sports are also investigated.

      Many college students experience daytime sleepiness. Daytime sleepiness is reportedly associated with poor academic performance, suicide, depression, and traffic accidents. This parallel-group randomized controlled trial study aimed to examine the effect of self-care using transcutaneous vagus nerve stimulation (taVNS) in college students with sleepiness and depression. Thirty-four college students (age 21.5 +/- 0.1 years, 21 men and 13 women) were assigned to a control or taVNS group. For taVNS, an electro-stimulator was used to deliver 100 Hz pulses at an intensity below each participant's pain threshold (3.0-8.0 mV) for 30 min. taVNS was performed 30 min before bedtime every day for 28 days. Daytime sleepiness, depression, study engagement, and sleep quality were measured using the Japanese version of the Epworth Sleepiness Scale (ESS-J), Japanese version of the Beck Depression Inventory-II (BDI-II-J), Japanese version of the Utrecht Work Engagement Scale for Students (UWES-S-J), Japanese version of the Pittsburgh Sleep Quality Index (PSQI-J), and Silmee (TM) Bar Type Lite. Autonomic nervous system activities were measured using high-frequency (HF) and low-frequency (LF) components of heart rate variability. Parasympathetic and sympathetic nervous system activities were indicated by the HF value and LF/HF ratio, respectively. The change in each group from day 28 to day 1 was calculated and analysed. The ESS-J, BDI-II-J, UWES-S-J, and PSQI-J (sleep quality and daytime dysfunction) scores improved, and the LF/HF ratio decreased in the taVNS group compared with the control group. Self-care using taVNS suppresses the sympathetic nervous system during sleep, which may improve sleep quality, daytime sleepiness, depression, and study engagement.

      Abstract

      This systematic review aimed to identify sports injury surveillance systems (ISSs) and injury surveillance guidelines (ISGs) developed and currently used in Japan. Additionally, the extent to which ISSs and ISGs developed overseas are employed in Japan was also examined. A systematic search of electronic databases was performed in March 2021. Prospective studies that investigated sports injuries and targeted Japanese athletes or sports in Japan were included to identify the following: (1) details of the ISSs or ISGs developed among sports in Japan, and (2) characteristics of studies focusing on sports in Japan employing ISSs or ISGs developed overseas and the method of use. A total of five ISSs developed among sports in Japan were included, most of which were based on insurance benefits or condolence payment systems. In total, 23 studies used ISSs or ISGs developed overseas, more than half of which were used as a resource for the development of other data collection tools or methods, indicating that the definition of injury and/or classification of items are modified based on the needs among sports in Japan. This review highlights the necessity of establishing club-based or organisational ISSs and ISGs that are appropriate for sports in Japan.

      Stability training is often carried out in the field of sports to improve performance. Appropriate loading is important in sports rehabilitation. Rehabilitation that ignores progressive characteristics can lead to a variety of reactions, including prolonged return to work and recurrence of injury. Therefore, it is desirable to objectively evaluate the load exerted by each training method. However, the degree of change in muscle activity in response to changes in foot ground contact height and stability when performing front bridge was unknown. Thirty-seven healthy subjects performed six different front bridges with varying foot height and instability. Muscle activity was measured and examined during implementation. Surface electromyography was used to measure the activity of the upper trapezius, lower trapezius, serratus anterior, rectus abdominis, external and internal abdominal oblique muscles, rectus femoris, and tibialis anterior. The rectus abdominis, external oblique abdominis, and internal oblique abdominis muscles showed significantly higher activity under conditions of instability of the foot ground contact surface. The upper trapezius, serratus anterior, external oblique abdominis, and tibialis anterior muscles showed significantly higher activity under conditions with a high foot ground contact surface. The upper trapezius, serratus anterior, external oblique abdominis, and tibialis anterior muscles required significantly higher muscle activity under high foot ground contact conditions. It was also suggested that the trunk muscles required significantly higher muscle activity under conditions where the foot ground contact surface was unstable.

      The Oslo Sports Trauma Research Center Questionnaire is an athletic epidemiological survey questionnaire targeting “all complaint injuries” (all problems with symptoms). The questionnaire includes questions regarding (1) participation, (2) modified training/competition, (3) performance, and (4) pain (symptoms) due to an injury at specific location or health problems at certain intervals. The prevalence and severity of problems are calculated to monitor changes in these parameters. This paper provides an overview of this questionnaire, its procedures, and major previous studies.

      BACKGROUND: Exercise therapy is the first choice non-pharmacotherapeutic approach for musculoskeletal pain; however, it often interferes with the implementation and continuation of exercise due to fear-avoidance behaviors. Trigger point acupuncture (TrPAcp) has been reported to reduce musculoskeletal pain. OBJECTIVES: To examine the efficacy of exercise combined with TrPAcp compared to exercise alone for older patients with chronic low back pain (CLBP), the most common subjective symptom reported by old people of both sexes in Japan. METHODS: In this single-center randomized controlled trial conducted at Teikyo Heisei University, 15 men and women aged ≥ 65 years with low back pain for at least 3 months who met the eligibility criteria were included. The Ex+TrPAcp group received exercise and trigger point acupuncture, while the Ex group received only exercise for 3 months. The main outcome, pain intensity, was measured using the numerical rating scale (NRS). Improvement was defined as a decrease in NRS of ≥ 2 or less than moderate (NRS < 4). RESULTS: The analysis included 7 of 8 cases in the Ex+TrPAcp group and 7 of 7 cases in the Ex group. NRS improved in 6 of 7 and 1 of 7 patients in the intervention and control groups, respectively, with a significant difference between groups (p = 0.03, ϕ = 0.71). Regarding adverse events due to acupuncture, one patient (14.3%) complained of heaviness after acupuncture. Nothing specific was reported with exercise. CONCLUSION: Compared with Ex alone, Ex+TrPAcp may be more effective therapy for older people with CLBP who do not have an exercise habit.

      臨床鍼灸総合実習を履修した本学鍼灸学科3年生67名を対象とし、直接患者に対して施術を行うに足る総合的知識および基本的技能・態度を備えているか評価するため、「基本的はり実技の試験」「基本的きゅう実技の試験」「医療面接・身体診察・臨床推論の試験」「知識および問題解決能力を評価する客観試験」を行った。結果、「基本的はり実技の試験」と「基本的きゅう実技の試験」の成績は、後者で再試験になった学生が1.5%いたが概ね高い評価得点であった。「医療面接・身体診察・臨床推論の試験」の成績は、全7項目の平均得点が6点満点で3.94点であった。「知識および問題解決能力を評価する客観試験」の成績は、平均正答率が42%で、国家試験の合格基準である60%とは18%の差があった。「授業のねらい及び到達目標」の到達度に係る総合評価を行い、その得点が妥当であったか検証するため、1年前期から3年後期までの通算Grade Point Averageとの相関係数を求めたところ、p<0.05で有意となり、相関係数r=0.69でかなり相関があった。

      BACKGROUND: Epidemiological data on sports injuries and illnesses depend on the surveillance methodology and the definition of the health problems. The effect of different surveillance methods on the data collection has been investigated for overuse injuries, but not for other health problems such as traumatic injuries and illnesses. PURPOSE: The purpose of this study was to investigate the new surveillance method developed by the Oslo Sports Trauma Research Center (OSTRC), which is based on any complaint definition (new method), to identify health problems compared with the traditional surveillance method, which is based on time loss definition. STUDY DESIGN: Descriptive epidemiology study. METHODS: A total of 62 Japanese athletes were prospectively followed-up for 18 weeks to assess differences in health problems identified by both new and traditional methods. Every week, the athletes completed the Japanese version of the OSTRC questionnaire (OSTRC-H2.​JP), whereas the teams' athletic trainers registered health problems with a time loss definition. The numbers of health problems identified via each surveillance method were calculated and compared with each other to assess any differences between their results. RESULTS: The average weekly response rate to the OSTRC-H2.​JP was 82.1% (95% CI, 79.8-84.3). This new method recorded 3.1 times more health problems (3.1 times more injuries and 2.8 times more illnesses) than the traditional method. The difference between both surveillance methods' counts was greater for overuse injuries (5.3 times) than for traumatic injuries (2.5 times). CONCLUSIONS: This study found that the new method captured more than three times as many health problems as the traditional method. In particular, the difference between both methods' counts was greater for overuse injuries than for traumatic injuries. LEVEL OF EVIDENCE: 2b.

      BACKGROUND: Although it is evident that the risk of injury in handball varies according to player position, it is unclear whether the characteristics of these injuries also differ. The aim of this study was to characterize and compare the position-associated injury profiles of university handball players in Japan. METHODS: A total of 2056 participants who played in the 2018 and 2019 Japanese National University Handball Championships were included in this cross-sectional study. A self-reported injury questionnaire regarding demographic and injury-related information was administered. RESULTS: The overall prevalence of injury during the one-year period preceding the questionnaire was 44.1%, and back (47.9%) and line (49.2%) players exhibited a higher proportion of injuries than players in other positions (P<0.001). The ankle, knee, and shoulder/clavicle were the most common body regions affected by traumatic injuries, and knee and shoulder/clavicle injuries differed based on player positions (P=0.033). Conversely, overuse injuries predominantly affected the lumber spine/lower back, knee, and foot/toe, although there were no significant differences between player positions (P=0.188). In terms of injury mechanism, line players exhibited a higher proportion of contact injuries and goalkeepers had a higher proportion of non-contact injuries than that of other positions (P<0.001). CONCLUSIONS: Back and line players experienced more injuries than players in other positions. Depending on the player position, there were significant differences in the prevalence and patterns of injury. Therefore, injury prevention measures need to consider the characteristics of injuries based on player positions.

      To evaluate whether financial aid for acupuncture therapy is beneficial for non-manufacturing job workers (office workers) who are aware of reduced job performance due to health issues (presenteeism), a four-wk pragmatic multicenter randomized controlled trial was conducted with office workers who were aware of their presenteeism. The control group only implemented the workplace-recommended presenteeism measures, whereas the intervention group received financial aid for acupuncture therapy of up to 8,000 JPY (Japanese yen) in addition to implementing the presenteeism measures recommended by each workplace. The major outcome measure was the World Health Organization Health and Work Performance Questionnaire relative presenteeism score. A total of 203 patients were assigned to the intervention (n=103) and control (n=108) groups. The intervention group underwent a median of 1.0 (interquartile range [IQR], 1.0 to 2.0) sessions of acupuncture for neck disorders (64%), back disorders (16%), and depressed mood/anxiety/irritation (5%), among others. Results showed that the intervention group had slightly better job performance than the control group (effect size [r]=0.15, p=0.03).       The aim of this study was to examine the injury profile of Japanese university handball players. In this cross-sectional study, a total of 1017 participants who played in the 2018 Japanese National University Handball Championship were followed. The incidence within the previous year was 0.59 [95% CI, confidence interval: 0.56-0.62] per player per year. The ankle (33.3%), knee (23.6%), and shoulder/clavicle (12.6%) were the body regions most affected by traumatic mechanisms, while the lumbar spine/lower back (26.0%), knee (15.7%), and shoulder (15.0%) were the body regions most affected by overuse mechanisms. Sprain (30.7%), ligamentous rupture (23.2%), and fracture (11.8%) were the main types of traumatic injuries, and stress fracture (25.0%) and lesion of meniscus or cartilage (25.0%) were the main types of overuse injuries. Female players were 1.5 times more likely (OR, odds ratio: 1.55 [95% CI: 1.20-2.01]) to experience an injury than their male counterparts, and line players were also 1.5 times more likely (OR: 1.49 [95% CI: 1.00-2.21]) to experience an injury than their wing counterparts. This study showed that there is a need for increased injury prevention efforts in Japanese handball, especially among female players.

      Background: A forefoot strike (FFS) could be a safer landing technique than a rearfoot strike (RFS) during a cutting motion to prevent anterior cruciate ligament (ACL) injury. Purpose: This study aimed to determine the joint angles, ground reaction force (GRF), and muscle activity levels associated with FFS and RFS landings during 180° turns. Study design: Cross-sectional study. Methods: Fourteen male soccer players from the University of Tsukuba football (soccer) club participated in this study. The FFS consisted of initial contact with the toes on the force plates followed by the rearfoot; meanwhile, the initial contact was performed with the heels on the force plates followed by the forefoot for the RFS. Ankle, knee, and hip joint angles were recorded using a three-dimensional motion capture system. GRFs were measured using a force plate. Gluteus medius (GM), rectus femoris (RF), vastus medialis (VM), vastus lateralis (VL), semitendinosus (ST), biceps femoris (BF), tibialis anterior (TA), and lateral gastrocnemius (GL) activities were measured by electromyography. Results: The activities of GM, GL, and ST from initial contact to early periods during landing into the ground with the FFS are larger than those with RFS. In addition, the results showed significant differences in lower-limb angles and GRFs between the FFS and RFS. Conclusion: These results suggest that there might be differences in ACL injury risk during a 180° turn between the FFS and the RFS pattern. An investigation into the grounding method that prevents injury is necessary in future studies. Levels of Evidence: Level 3b.

      [Objective]To investigate the effects of acupuncture on sports activities of athletes
      [Methods]A total of 1,804 athletes were targeted. The method used was a collective survey. Questionnaires were administered after obtaining consent from the participants; online questionnaires were also provided. The questionnaire evaluated profile (sex, age); sensation immediately after acupuncture treatment, duration of stimulus sensation, stimulus intensity, stimulus sensation, and therapeutic effect; timing of acupuncture treatment; and questions related to acupuncture as to whether the acupuncture caused any adverse effects on play (movement) or showed any positive effects.
      [Results]In total, 1,525 (84.5%) valid responses were obtained. The survey analyzed 841 subjects (55.1%) who underwent acupuncture. The most common response after acupuncture treatment was that the subjects felt lighter and were able to move more easily; 43.9% of subjects gave this response. In addition, 10.6% of respondents said that they continued to feel pain similar to the sensation during acupuncture treatment and 7.7% said they felt weak. The most common response at 59.8% was that the sensation felt during acupuncture disappeared quickly after treatment was ended, 58.6% of subjects said that the sensation felt after acupuncture was optimal, and 48.3% responded that the sensation from treatment remained and a positive effect was observed. The most common timing for receiving acupuncture for 21.8% of subjects was after practice or a match:
      [Discussion and Conclusion]Acupuncture for athletes should be performed according to the guidelines, taking into account the competition schedule and type of competition and the appropriate amount of acupuncture stimulation, thereby preventing problems during competition.

      This study aimed to examine lower limb kinematics during a side-cutting task in male collegiate soccer athletes with severe ankle laxity. Forty-seven participants with a history of ankle sprains and perceived ankle instability were categorized into non-laxity (n = 17), laxity (n = 19), and severe laxity (n = 11) groups using stress radiography tests. Three-dimensional kinematic data during the stance phase of a 45° side-cutting task were analysed. The frontal plane kinematics of the knee significantly differed between the three groups (p < 0.05). The severe laxity group exhibited a greater abduction angle than the non-laxity group (p < 0.05). The horizontal and sagittal plane kinematics of the rearfoot differed between the three groups during the end of the stance phase (p < 0.05). Our data suggest that collegiate soccer athletes with both perceived ankle instability and severe ankle laxity exhibit greater knee abduction movement during a 45° side-cutting task compared to those with only perceived ankle instability.

      OBJECTIVE: Although the incidence and pattern of injuries in youth handball have widely been investigated, it is unclear whether the characteristics of injuries differ on the basis of player position. This study aimed at revealing the player position-based injury profile in Japanese youth handball players. DESIGN: Cross-sectional. SETTING: Injury questionnaire. PARTICIPANTS: Overall, 2377 participants who played in the 2018 and 2019 Japanese National High School Handball Championships were included. MAIN OUTCOME MEASURES: Prevalence, body region, type, severity, and mechanism of injuries were collected through a questionnaire. RESULTS: The overall prevalence of injury within the previous year was 46.7%, and the proportion of back players (52.8%) who sustained at least one injury was higher than that of players in other positions (p < 0.001). Differences in player positions were found among overuse injuries by body region, with a higher proportion of the lower leg and knee in wings and backs, respectively (p = 0.047). Among traumatic injuries, a higher proportion of ligamentous rupture in backs and fracture in lines were seen (p = 0.011). CONCLUSION: There were significant differences in the prevalence and patterns of injuries between player positions. Therefore, injury prevention programmes should consider the characteristics of player positions.

      橈骨遠位端骨折非転位例における補助診断として、単純X線写真でのpronator quadratus sign(以下PQS)がある。PQSとは前腕遠位掌側における方形回内筋の表層に位置した脂肪層(fat pad)が骨折などの出血で浮き上がる兆候である。本研究の目的は超音波診断装置を用いて、PQSを同定し描出することである。対象者は、上肢に外傷の既往がない大学生7名(19.1±0.4歳)の右7手とした。右手関節部側面像の単純X線写真の撮影を行いPQSを同定したあと、同部に超音波診断装置aplioMX(TOSHIBA製)を用いて、PQSを撮像できるか検討した。全対象者にPQSの撮像が可能であった。前腕回内回外中間位が一番描出しやすいが、前腕回外位でのPQSの描出は5名(71.4%)が低エコー像となり、2名(28.6%)が高エコー像であった。(著者抄録)

      Monitoring the health of athletes is important for their protection, and questionnaires such as those produced by the Oslo Sports Trauma Research Center (OSTRC) are a valuable tool in this process. In 2020, several changes were made to the OSTRC questionnaires (OSTRC-O, OSTRC-H), including changes to the wording, structure, and logic of the original questionnaires. In the present study, the Japanese versions of the OSTRC questionnaires (OSTRC-O.JP, OSTRC-H.JP) were revised to meet the requirements of the updated versions and to analyse new and previously collected data to illustrate the impact of the changes on Japanese athletes. Proposed changes were categorized as minor or more substantial; minor changes were effected to the questionnaire instructions and to the wording of all four questions, and more substantial changes were made to the wording of question 2. The updated questionnaires also included changes to questionnaire logic and answer categories. To assess the consequences of the changes to the wording of question 2, 101 athletes were asked to complete the OSTRC-H.JP, which included both the original and updated versions of question 2, over 10 consecutive weeks. We calculated the number of health problems identified when new gatekeeper logic was and was not applied, using 1585 OSTRC-H.JP responses to assess the consequences of the changes to the questionnaire logic. The kappa coefficient, which measures the level of agreement between the responses to question 2 of the original and updated versions, was high. By applying gatekeeper logic, there was a remarkable reduction in the number of injuries and illnesses among all health problems but less reduction in substantial health problems and time loss health problems. These changes will make it easier for Japanese athletes to complete the questionnaires and improve the quality of collected data.

      OBJECTIVES: To investigate whether ankle joint laxity alone influences lower limb kinematics during a side-cutting task. DESIGN: A cross-sectional study. SETTING: Sports medicine research laboratory. PARTICIPANTS: In total, 66 male collegiate soccer players with history of ankle sprains with no perceived ankle instability were categorised into three groups: no-laxity copers (n = 26), laxity copers (n = 23), and severe-laxity copers (n = 17). MAIN OUTCOME MEASURES: The hip, knee, ankle, rearfoot, midfoot, and forefoot kinematic data during the stance phase (0%-100% indicated initial contact to take-off) of a 45° side-cutting task were analysed using one-dimensional statistical parametric mapping. RESULTS: The horizontal plane kinematics of the rearfoot differed significantly among the three groups during 30%-91% of the stance phase (P < .05). Severe-laxity copers exhibited a greater external rotation angle than no-laxity copers during 6%-14% and 32%-92% of the stance phase (P < .05). CONCLUSION: Our data suggest that severe ankle joint laxity affects rearfoot horizontal plane kinematics in individuals without perceived ankle instability performing a 45° side-cutting task. These findings could be used by clinicians in developing rehabilitation programs to prevent further ankle sprains in patients with severe ankle joint laxity.

      BACKGROUND: Understanding lower limb kinematics and postural control in different directions of single-leg landings is critical to evaluate postural control and prevent lower limb injuries. However, foot and ankle kinematics and postural control during single-leg landings in different directions are less known. RESEARCH QUESTION: Does the difference in the direction of single-leg landing affect the foot kinematics on the frontal plane and dynamic postural stability? METHODS: A cross-sectional study was conducted. Forty-nine male collegiate soccer players performed single-leg forward (FL), 45° lateral (LL), and medial (ML) direction landings. The lower limb, foot (rearfoot, midfoot, forefoot), and ankle kinematics during an impact phase were evaluated, and a curve analysis was performed using a statistical parametric mapping method to compare the three landings. The three landings were compared in terms of postural control parameters, including time to stabilization (TTS), peak of ground reaction forces (GRFs), root-mean-square of the mediolateral GRFs for 0-0.4 s (GRFML0.4), loading rate, and magnitude of horizontal GRFs from 0-0.4 s (HGRF-0.4), 0.4-2.4 s (HGRF-2.4), and 3.0-5.0 s. RESULTS: Ankle and rearfoot kinematics in LL exhibited smaller eversion and pronation positions than FL and ML (p < 0.01). The TTS-mediolateral (TTS-ML) was longer in the LL than in FL and ML (p < 0.001). The GRFML0.4, HGRF-0.4, and -2.4 in the LL and ML were greater than those in the FL (p < 0.001). SIGNIFICANCE: Directions of single-leg landing affect foot and ankle kinematics and postural stability. Specifically, the LL exhibits more inverted ankle and supinated rearfoot positions, and longer TTS-ML. Thus, the LL may induce stretching of the lateral ankle ligament. These findings can help understand foot kinematics and assess dynamic postural control.

      [Objective] The purpose of this cross-sectional questionnaire-based study is to investigate the challenges of adverse events during treatment by acupuncture and moxibustion to an athlete.
      [Methods] In total, 1,804 subjects, including athletes who participated in marathons and university athletes were enrolled in this study. The survey was conducted by distributing the questionnaire or via online questionnaire to the athletes who fit the criteria for the study. The questionnaires were collected after consent was received from the participants; online questionnaires were also collected. The questionnaire was examined for age, sex, and type of competition, purpose of the acupuncture treatment and the actual effects, and treated area, content of adverse event, number of times the subjects received acupuncture, and informed consent, and free description of adverse events.
      [Results] In total, 1,525 (84.5%) valid responses to the questionnaires were received. The survey analyzed 841 subjects (55.1%) who experienced acupuncture and moxibustion. The following responses about adverse events of acupuncture were obtained: "pain at time of needle insertion"; 17.0%, "subcutaneous-bleeding"; 10.1% and "pain and feeling of heaviness due to the needle": 10.0%, among others. For moxibustion, responses included "burns"; 5.9% and "malaise"; 5.8%, among others. In the free description, there were 17 descriptions of discomfort due to excessive stimulation and difficulty in movement during exercise. Of the questions regarding whether or not they received informed consent, 215 were received orally (25.6%), 34 were received in written form (4.0%), 99 were received both orally and in written form (11.8%).
      [Discussion and Conclusion] Acupuncture for athletes causes a certain rate of adverse events. It was suggested that guidelines for athletes to receive acupuncture safely and training and education for practitioners are important.

      This study aimed to translate and culturally adapt the Oslo Sports Trauma Research Center Overuse Injury Questionnaire (OSTRC-O) and the Oslo Sports Trauma Research Center Questionnaire on Health Problems (OSTRC-H) into the Japanese context. The validity and reliability of these translated questionnaires examining overuse injuries and health problems among Japanese university athletes were also examined. The translation was performed following an internationally recognized methodology. A total of 145 athletes were tracked over 10 consecutive weeks and four questions were added in the 10th week to examine the questionnaires' content validity. Test-retest analysis for reliability was performed 24-72 hours after the 10th week of registration. Internal consistency was determined by calculating Cronbach's a during the cohort study. No major disagreements were found in the translation process. The translated questionnaires had high acceptance and compliance, with an average response rate of over 80% throughout the 10-week cohort study. Most participants reported that the questionnaires were not difficult to complete, there were no items they wanted to change or add, and that the web-based technique worked effectively. Good test-retest reliability and high internal consistency was observed in the translated questionnaires. The translated questionnaires were found to be valid, reliable, and acceptable for medically monitoring Japanese athletes.

      [Background] Presenteeism, defined as the practice of sick workers who come to work, and whose work performance effectiveness has been reduced due to various signs and symptoms, causes a significant financial loss to companies. However, comprehensive and effective occupational health countermeasures have not yet been presented. Therefore, we report here the results of an interim analysis to determine whether an allowance for acupuncture treatment is beneficial for workers' presenteeism.
      [Methods] A four-week randomized intergroup comparative study was conducted on office workers who were aware of their presenteeism. Participants were allocated to either the control group, in which regular recommended presenteeism countermeasures for each workplace were implemented arbitrarily, or to the intervention group in which an allowance of up to 8,000 was given for acupuncture treatment, in addition to the usual arbitrary measures. The primary endpoint was the WHO-HPQ relative presenteeism score: a score lower than 1 indicates lower work performance; and it was analyzed with the "full analysis set" population.
      [Results] A total of 52 patients were assigned to the intervention group (n = 30) and the control group (n = 22). On average, the intervention group received acupuncture treatments 1.4 times for stiff neck and shoulders (67%), lower back pain (26%), depression (5%), and allergies (2%), and paid a total of 7,219; and 6,556 was paid as an expense allowance. As a result, the relative presenteeism score was 0.95 in the intervention group, compared to 0.91 in the control group, with a between-group difference of 0.04 (ES (r) = 0.22, P = 0.12).
      [Conclusions] With a total of up to 8,000 offered to office workers who were aware of their presenteeism, an acupuncture treatment allowance for a four-week period was provided 1.4 times on average. The results suggested that the treatment allowance would increase workers' performance efficiency by about 4% (equivalent to 19,691 per person) compared to those without it.

      OBJECTIVE: To examine the lower limb kinematics and muscle activities in diagonal single-leg rebound jump in soccer players with chronic ankle instability (CAI). DESIGN: Cross-sectional study. SETTING: Laboratory. PARTICIPANTS: Thirty male collegiate soccer players participated: 15 with CAI were compared with 15 without CAI, matched by physical description. INTERVENTION(S): In the diagonal single-leg rebound jump, participants stood on one leg on a 30-cm high box, hopped down diagonally (45°) onto a force plate, and jumped vertically as high as possible with hands on their hips. MAIN OUTCOME MEASURES: Hip, knee, and ankle kinematics were acquired using a motion capture system. The activity of the gluteus medius, hip adductor, and lower leg muscles was recorded using electromyography. Jump performance was calculated using a force plate. RESULTS: The CAI group had (i) decreased hip adduction, knee flexion, external rotation, and dorsiflexion angle; (ii) reduced hip adductor and peroneus muscle activations; and (iii) reduced jump height and short flight time. CONCLUSIONS: Male collegiate soccer players with CAI showed altered kinematics and muscle activities during a diagonal single-leg rebound jump; this may adversely affect rebound jump performance.

      OBJECTIVE: The purpose of this study was to investigate the association between Cumberland Ankle Instability Tool (CAIT) scores and postural stability during a diagonal landing, and to investigate whether postural stability is altered in collegiate soccer players with and without functional ankle instability (FAI). DESIGN: Cross-sectional study. SETTING: Laboratory. PARTICIPANTS: Ninety-one soccer players were classified into a FAI group (history of at least two ankle sprains and a CAIT-Japanese version score ≤25, n = 28), a copers group (history of one ankle sprain and a CAIT-Japanese version score ≥26, n = 32), or a control group (no history of ankle sprain, n = 31). MAIN OUTCOME MEASURES: Time to anteroposterior stabilisation (TTSAP) and mediolateral stabilisation (TTSML) were measured during the diagonal single-leg landing. RESULTS: The CAIT scores were correlated with TTSAP (P < 0.05, rs = -0.214) and TTSML (P < 0.01, rs = -0.566). TTSAP was longer in the FAI group than in the control group, and TTSML was longer in the FAI group than in the other groups. CONCLUSIONS: Our findings indicate the presence of an association between the CAIT-J score and TTSML, as well as postural stability deficits in collegiate soccer players with FAI during diagonal landings.

      BACKGROUND: The autonomic nervous system and trigeminal nerve are involved in adjusting flow through diverging cerebral arteries in the prefrontal cortex. The purpose of this study was to examine the effect of 100 Hz electroacupuncture (EA) to the trigeminal nerve area on cerebral blood flow and autonomic nervous system function. METHOD: This was a randomised crossover study of 16 healthy volunteers who were assigned to an EA or control group. Stimulation (in the EA group) was performed five times, each after 1 min of rest. Needles were inserted at the inner edge of the eyebrows and 1 cm from the front hairline midpoint. We used high-frequency (HF) and low-frequency (LF) components of heart rate (HR) variability to assess autonomic nervous system function. HF and LF/HF ratio were taken as indicators of parasympathetic and sympathetic nervous system activity, respectively. We measured cerebral blood flow using a two-channel near-infrared spectroscope. RESULTS: In the EA group, HR significantly decreased (p=0.004) and HF significantly increased (p=0.006) relative to baseline. By contrast, there were no significant changes in HR or HF within the control group (p>0.05). Accordingly, HR tended to be lower (p=0.087) and HF greater (p=0.071) in the EA group versus the control group. There were no significant differences in LF/HF ratio within/between groups. Compared with the control group, cerebral blood flow was significantly greater in the left (p=0.048) and right (p=0.016) prefrontal cortex in the EA group. CONCLUSIONS: Delivery of 100 Hz EA to the trigeminal nerve area reduces HR and increases parasympathetic nervous activity and cerebral blood flow.

      BACKGROUND: Single-legged drop jumps (SDJ), single-legged repetitive jumps (SRJ), and single-legged side hops (SSH) are often used as plyometric training and functional performance tests. Differences in the kinetics and kinematic characteristics of lower extremity joints during these jumps are unclear. HYPOTHESIS/PURPOSE: The purpose of this study was to investigate the joint motion and mechanical work of the takeoff leg from foot contact to foot-off during SDJ, SRJ, and SSH in the sagittal and frontal planes in female athletes. It was hypothesized that the joint motion and mechanical work of the lower extremity joints during the SDJ and SRJ would be larger than the SSH in the sagittal plane, those during the SSH would be larger than the SDJ and SRJ in the frontal plane, and during SRJ would be larger than SDJ. STUDY DESIGN: Cross-sectional study. METHODS: Seventeen female collegiate athletes participated and performed the SDJ (0.15-m box height), and SRJ and SSH (by crossing two lines 0.3 m apart). Three-dimensional coordinate data and ground reaction forces were collected. Contact time, jump height, jump index (i.e., the jump height divided by the contact time) of the SDJ and SRJ, and the total times of the SSH were calculated. Range of motion (ROM) from touchdown to the lowest center of mass, and the positive and negative (mechanical) work from touchdown to foot-off were analyzed. RESULTS: There were no significant differences in jump performance variables. Compared to the SSH, the SDJ and SRJ had significantly larger ankle and knee ROM and positive and negative work at the lower extremity joints, except for positive work at the hip joint, in the sagittal plane (p < 0.05). Compared to the SDJ and SRJ, the SSH had a significantly larger ankle ROM and positive work at the knee joint in the frontal plane (p < 0.05). Compared to the SDJ, the SRJ had a significantly larger ROM and negative work at each lower extremity joint in the frontal plane (p < 0.05). CONCLUSION: Although there were no significant differences in the jump performance variables, different characteristics of the takeoff leg ROM and mechanical work were found between three kinds of rebound-type jump tests. These findings may help clinicians choose jump methods to assess lower extremity function and to design plyometric training programs in sports and clinical fields. LEVEL OF EVIDENCE: 3b.

      32名の健康な若年成人男性(平均24.32±2.47歳)を鍼刺激群と対照群に分け、両群に30回の等尺性最大随意収縮から成る消耗運動を行わせた。鍼刺激群には消耗運動前に円皮鍼刺激を行った。消耗運動の前・後に等尺性最大膝伸筋力、筋力の立ち上がり率、力の時間積分値と筋活動(積分筋電図と平均パワー周波数)を測定し、円皮鍼刺激の効果を評価した。二元配置分散分析を行ったところ、介入の主要な効果と、介入×時間相互作用の主要な効果は、最大筋力、筋力立ち上がり率、力の時間積分値と積分筋電図に関して有意であった。以上より、円皮鍼は運動による筋肉疲労を防ぐことが出来ると考えられた。

      BACKGROUND: A previous study has reported that low-frequency (LF) electroacupuncture (EA) influences salivary secretory immunoglobulin A (sIgA) and the autonomic nervous system (ANS). The ANS is known to control the secretion volume of sIgA; however, the effect of high-frequency (HF) EA on salivary sIgA has not been determined. We investigated whether HF EA affects salivary sIgA levels and the ANS. METHOD: Sixteen healthy subjects were randomly classified into two groups: a control group and an EA group. After a 5 min rest, subjects in the EA group received EA at 100 Hz bilaterally at LI4 and LI11 for 15 min before resting for a further 40 min post-stimulation. Subjects in the control group rested for a total of 60 min. Measurements of the ANS and sIgA levels in both groups were made before, immediately after, 20 min after, and 40 min after rest or 15 min EA treatment. HF and LF components of heart rate variability were analysed as markers of ANS function. LF/HF ratio and HF were taken as indices of sympathetic and parasympathetic nerve activity, respectively. Salivary protein concentrations and sIgA levels were determined by Bradford protein assay and ELISA, respectively. RESULTS: LF/HF ratio was significantly increased immediately after EA. HF was significantly increased at 20 min after EA and sIgA level was significantly increased at 40 min after EA. In addition, HF and salivary sIgA level were positively correlated with each another. CONCLUSIONS: HF EA exerted sequential positive effects on sympathetic nerve activity, parasympathetic nerve activity, and salivary sIgA level (immediately and after 20 and 40 min, respectively). HF EA may increase salivary sIgA levels by influencing parasympathetic nerve activity.

      スポーツ選手がパフォーマンスを十分に発揮するためには、傷害を未然に防ぎ、コンディションを整える必要があることは言うまでもない。しかし、コンディションに影響する因子は、身体面のみならず、環境面、心理面など多岐にわたっており、その全てを適時的確に把握するための科学的根拠は数少ない現状にある。そこで、本研究の目的は、スポーツ選手のコンディションを適時的確に把握する方法を探るため、「コンディション登録データベース」を構築して経時的にデータを集積し、①大学サッカー選手の身体的問題と心理的問題の発生状況と関連性、②身体的問題と心理的問題が選手のパフォーマンスに与える影響を明らかにすることである。
      本研究の目的を達成するため、横断研究として、大学サッカー選手を対象に、身体的・心理的問題の発生状況をWEBアンケートにより調査し、分析する。また、縦断研究として、身体的・心理的問題の発生状況の推移に併せて、選手のパフォーマンス状況の推移を2年間継続的に評価し、分析する。これにより、スポーツ選手のコンディションを適時的確に把握する方法を確立することを目的とし、大学サッカー選手において、身体的問題と心理的問題の発生がパフォーマンスに影響するという仮説を検証する。
      2021年度においては、横断研究として、1000名を超える全国の大学サッカー選手を対象として、身体的・心理的問題の発生状況についてのWEBアンケートを実施し、回答を得られた。また、縦断的研究として、大学サッカー選手のパフォーマンスについて継時的に評価を継続しており、今後はこれらの関連性の検討を行う予定である。

      In the prevention of anterior cruciate ligament (ACL) injuries, the forefoot strike (FFS) could be a safer landing technique than the rearfoot strike (RFS) during cutting motions. In this study, we aimed to clarify the joint angles, ground reaction forces (GRFs), and muscle activity levels associated with FFS and RFS landings during 180°turns. Our results showed significant differences in the lower-limb angles, muscle activities, and GRFs between the FFS and RFS. These findings suggest that increased lateral gastrocnemius (GL) and semitendinosus (ST) activities during the FFS may prevent large amounts of tension from being placed on the ACL and thereby reduce the risk of injury. Furthermore, we investigated the injury rates before and after giving out the instructions regarding the forefoot landing movement. However, no significant difference was noted between the injury rates before and after instruction.

      The aim of this study was to investigate the relationship between the muscle actibity and the risk of anterior cruciate ligament (ACL) injury.
      At 50 ms prior to ground contact, BF, ST, and GL muscle activity was significantly greater during FFS cutting than during RFS cutting, and TA muscle activity was significantly greater during RFS cutting. At 50 ms after ground contact, BF and GL muscle activity was significantly greater during FFS cutting than during RFS cutting, while TA muscle activity was significantly lower during FFS cutting than during RFS cutting.
