    ABSTRACT

    Evidence‐based practice (EBP) is recognized as essential for improving patient outcomes and healthcare quality. Implementation leadership by nurse managers plays a crucial role in promoting EBP in hospital wards. However, the mechanisms by which implementation leadership is related to nurses' EBP activities remain unclear. This research, a secondary analysis of cross‐sectional data from three Japanese university hospitals, included 473 nurses from 84 wards. Implementation leadership, EBP beliefs, and EBP behaviors were measured using the Implementation Leadership Scale, EBP Beliefs Scale, and EBP Questionnaire‐Japanese version. Multilevel structural equation modeling showed that at the ward level, implementation leadership directly affected staff nurses' EBP behaviors. At the individual level, the relationship between implementation leadership and EBP behaviors was fully mediated by EBP beliefs. The intraclass correlation coefficient was higher for Implementation Leadership Scale scores (0.20) than for EBP Beliefs Scale (0.05) and EBP Questionnaire‐Japanese version scores (0.03), indicating that implementation leadership was assessed similarly within wards. However, individual factors primarily affected EBP beliefs and behaviors. These findings suggest the importance of ward‐level support and individual belief formation in promoting EBP implementation.

    Abstract

    Background

    The outbreak of COVID-19 in 2019 led governments worldwide to introduce various public health measures, which included restrictions on travel and public gatherings, effectively reducing the spread of the virus and associated mortality rates. In Japan, nonlegally binding restrictions on outings effectively curbed infections, as in other countries. However, the restrictions impacted lifestyles, including reduced physical activity, increased frailty, and overeating issues, beyond the effect of preventing the spread of infection. Various factors such as personality, age, and cultural norms influenced outing behavior during the pandemic, which varied by activity type.

    Objective

    To elucidate the association between personality traits and changes in outing behaviors during the COVID-19 pandemic, as well as to clarify age-specific differences in outing behaviors, focusing on different types of outings.

    Methods

    A cross-sectional survey was conducted using a web-based questionnaire in January 2021, when Japan announced its second emergency declaration during the pandemic. Overall, 1236 participants were recruited, with an equal number of participants for each gender and 10-year age group. The survey included questions regarding changes in the frequency of three types of outings—medical institution visits, eating out, and traveling—in addition to participants’ personality traits, such as sociability and morality. Multinomial logistic regression analysis was performed to analyze the association between personality traits and changes in different outing behaviors. Stratified analysis by age group was also performed.

    Results

    The findings revealed that 790 participants reported no change in medical institution visits, although the frequency of eating out and traveling decreased during the pandemic. Regarding an age-wise comparison, a higher percentage of older people reported no change in medical institution visits but reported a decrease in eating out and traveling than younger people. Multinomial logistic regression analysis stratified by age showed that sociable people were more likely to report a decrease in the frequency of medical institution visits and an increase in the frequency of eating out (odds ratio [OR] 1.92, 95% CI 1.36‐2.71, P<.001; OR 2.57, 95% CI 1.19‐5.54, P=.016, respectively), and participants with a strong sense of responsibility were more likely to report a decrease in the frequency of traveling (OR 1.76, 95% CI 1.14‐2.72, P=.011) among younger adults. Among older adults, strongly responsible individuals were less likely to eating out frequently (OR 2.56, 95% CI 1.12‐5.82, P=.026).

    Conclusions

    We examined various behavioral changes observed during the pandemic for different types of outings and their associations with personality traits, as well as differences between age groups. The findings could help promote an understanding of how to effectively communicate and engage in appropriate behaviors in public health emergency settings.

    Background. Strategic leadership is key to implementing evidence-based practice (EBP). Evaluating the readiness and processes necessary to implement EBP using the Japanese version of the implementation leadership scale (ILS) may be useful to systematically promote the implementation of EBP in Japan. This study aimed to evaluate the reliability and validity of the Japanese version of the ILS for nurse managers and staff nurses. Methods. Data were collected in a cross-sectional study. The original ILS was translated into Japanese and back-translated into English. Clinical nurses reviewed it and confirmed its face validity. We distributed a web questionnaire to 119 nurse managers and 2,858 staff nurses working at three university hospitals in Japan’s metropolitan areas. Construct validity was assessed for nursing managers and staff nurses, respectively, using confirmatory factor analysis. Known-group validity for nurse managers was assessed by verifying differences in ILS scores by educational background and experience of learning EBP and working on EBP. We evaluated reliability using Cronbach’s alpha and test-retest reliability. Results. The response rates for nurse managers and staff nurses were 56.3% and 16.9%, respectively. Data from 67 nursing managers and 484 staff nurses were analyzed, excluding duplicate responses. Confirmatory factor analyses of both samples supported the four-factor structure of ILS. The ILS total score of nurse managers with experience learning EBP or experience working on EBP was statistically significantly higher than that of those with no experience, and known-group validity was supported. Across both samples, internal consistency reliability was strong (Cronbach’s alpha: 0.91–0.97) and test-retest reliability was moderate. Conclusion. This study illustrated the reliability and validity of Japanese versions of the ILS for both nurse managers and staff nurses. This study enabled international comparisons of the leadership required for EBP implementation and may support the development of intervention programs and strategies to promote EBP’s implementation in different countries.     本研究の目的は、看護系大学で学ぶ学生の学生時代の経験、学修や生活が就職後の組織への適応に及ぼす影響を検討することである。
    昨年度、やや計画に遅れがあったため、本年度の研究計画を下記に修正した。
    ①先行研究のレビューを踏まえ、予備調査用の質問紙の作成をおこなう、②看護系大学を卒業後、病院に勤務し、１年の業務経験を有する看護師経験２年目の看護師100名を対象にWEB調査にて予備調査を実施する、③調査で得たデータを分析し、質問項目の精査、修正をおこなう、④調査設計の専門家等のスーパーバイズを受けたのち、本調査に向け質問紙の完成を目指す、⑤本調査への協力施設を募り、本調査の準備をおこなう、ということである。
    ⑤の具体的な内容は、病院要覧（全国病院名簿）に掲載されている100床以上の病院を病床数で層化し、無作為に100病院を抽出する。対象病院の看護部長宛に調査への協力可否等を記載した往復はがきを送付するというものであった。
    本年度は新型コロナウイルス感染症の感染拡大の影響を受け、対象者となる看護師の経験の特異性、調査対象者の疲弊感、調査協力の負担によるバイアス等を鑑み、調査を前向きに進めていくことが困難であった。
    よって、実績としては、①先行研究のレビューを踏まえ、予備調査用の質問紙の作成をおこなう、④作成した質問紙に対し、調査設計の専門家等のスーパーバイズを受けた、ということにとどまった。先行研究のレビューを丁寧におこなったことから、既存の大学から仕事へのトランジション研究で扱われていた測定用具だけではなく、広くトランジションに影響を与えうる概念を抽出することができた。

    We have developed an educational program that is not limited to the field of nursing education, nor is it limited to pre-departure education, but is based on the long-term perspective of lifelong learning. In other words, we established a prototype of a "medical education workshop using theater" to improve communication and empathy as a medical education program through pre- and post-graduates. We plan to continue to implement the program and evaluate the long-term outcomes.

    In nursing, nurses provide assistance to patients while they are performing various types of movement. Since safety is the first priority in helping patients, nurses tend to give assistance to them more than what is needed. The purpose of the present study was to identify the optimal way to assist sit-to-stand movement for both patients and nurses. The optimal way should be the one that applies the least amount of load to a nurse and that enables the patient to use their ability to a maximum extent. In order to create a simple model for load analysis, we assumed that the load during the assisted sit-to-stand movement was transferred between the nurse and patient at one site. Based on this assumption, we developed a custom-made 3D motion analysis system that measures the amount and direction of the load while the nurse is helping the patient to perform the sit-to-stand movement. We collected 3D motion data as well as load data and discussed what defines the “optimal” way.
